Follicular unit excision (FUE) is a hair transplant that requires “removing individual follicular units (natural clusters of hairs grouped in 1-4 hairs each) from the back of the head and at times from the beard and other body regions to be transplanted typically back into balding areas of the scalp or other areas of the body as aesthetically needed,” according to the International Society of Hair Restoration Surgery.

It is common to shave one’s head as part of the procedure, which can take anywhere from six months to a year to show full results.

In April 2025, Cena revealed that he’d undergone his first hair transplant.

“There’s another headline for ya: hair transplant,” he said on* The Pat McAfee Show* at the time. “There’s no shame in that, man. I got a problem, I tried to fix it. I’m working on it. I saw so many bald spot signs.”

The former WWE star spoke in depth about the decision in an interview with PEOPLE last August, revealing that he was propelled to look into treatment options after wrestling fans kept commenting on his bald spot.

“I saw their signs that said ‘The bald John Cena,’” he told the outlet. “They pushed me into going to see what my options were. I now have a routine: red-light therapy, minoxidil, vitamins, shampoo, conditioner — and I also got a hair transplant last November.”

He continued, “I hate the fact that if there wasn’t so much shame around it, I’d have gotten it done 10 years ago. I thought I was alone, but seven or eight out of 10 [men] suffer from thinning or baldness.”

Cena noted that he is comfortable with sharing his procedure story now, adding, “It completely changed the course of my life.”

He also provided an update on the current status of his hair in a recent interview with *Us Weekly*.

“My first transplant was a minimal procedure. I had a huge spot in the back, and they covered it up, but I didn’t exhaust my donor sites,” he said. “Now I’m talking inside baseball, but this is all knowledge I got by talking to a professional. Imagine what can happen!”

He added, “That’s why I’m such a strong advocate. I see [a doctor] every six weeks. I get PRP treatment. I do red-light therapy, vitamins, finasteride topical, exosome topical — I’m all in.”
