Taylor Swift has released “Patient Zero” as the lead single from her surprise album expansion, The Life of a Showgirl: The Encore

The song explores themes of toxic relationships and resilience, with Swift offering herself as someone who’s been through it

The expanded album features four new tracks written during a Sweden trip with collaborators Max Martin and Shellback

Taylor Swift is kicking off another Swiftie outbreak with “Patient Zero.”

Released on Friday, Sept. 25, Swift, 36, dropped her new single as the lead track from The Life of a Showgirl: The Encore, a surprise expanded edition of her 2025 album featuring four new songs, which she announced on Wednesday, Sept. 23.

The pop superstar first announced “Patient Zero” on Tuesday, Sept. 22, after teasing fans with a countdown and a series of clues.

“I’ve been impatiently waiting to tell you that my brand new single ‘Patient Zero’ will be out on September 25 (!!!!!!!!!!!!) and it’s available to pre-order now on my website for 24 hours,” Swift wrote on Instagram.

On Sept. 24, Swift gave fans a 13-second glimpse at her new “Patient Zero” music video, which stars Swift alongside Dakota Johnson and Colin Farrell. The clip, which aired exclusively on CBS Mornings Thursday, offers a brief look at the cinematic visual, including a shot of Swift sporting a notably pared-back look.

And Swifties, as usual, have already started looking for clues.

After Swift teased a “North or South” clue in her SUV Emmys cameo, fans quickly connected the hint to the 36-year-old Fifty Shades of Grey star. The full “Patient Zero” music video is set to premiere during the MTV Video Music Awards on Sunday, Sept. 27.

Ahead of the announcement, Swift changed the “o” letters in her Instagram bio to zeroes, writing, “And, baby, that’s sh0w business f0r y0u. ❤️‍🔥” The singer also appeared to make similar changes to lyrics displayed on Apple Music.

In the song, Swift appears to address a woman dealing with a charismatic but toxic man, offering herself as someone who has already been through the same situation.

“If you wanna party with somebody / who might know about the devil on his shoulder under his halo,” the lyrics read. “When the toxins take their hold / you’d rather die than let it go / But if you’re sick of him, I got you / I was patient zero.”

The narrator then makes it clear that she has some history with the man in question.

“I’ve gotta be honest, / I’ve been expecting your call / No, it’s not a bad time at all,” the lyrics continue. “I heard somebody saying there’s trouble in paradise / And my lips are sealed tight now / It’s not for me to tell someone else what to do.”

Later, she reveals that she’s been in the same position before — and walked away.

“Take it from somebody who lost his case but won the race to give them someone who said all of this to his face / someone who knows she’s more than just the chase / Someone who had him and walked away,” she sings.

The song’s title gets another explanation in the chorus: “So if you wanna kick it, then I’m with it, I’ve been there / Felt the reeling, done the healing, said the curses and prayers / When the fever turns a dreamer into a pathetic ghost / If he’s killing you, I see you / I was patient zero.”

“Patient Zero” is one of four new songs on The Life of a Showgirl: The Encore, which Swift announced the day after revealing the single. The expanded edition will also include “Cleveland!,” “Pink Clouding” and “Babylon.”

Swift explained that the new songs came out of a trip to Sweden with longtime collaborators Max Martin and Shellback, who also worked with her on The Life of a Showgirl.

“About a year ago, you guys did a truly unfathomable thing. Something that completely blew my mind… You gave The Life of a Showgirl the biggest first week for an album in history,” Swift wrote in her Instagram announcement. “I took a trip to Sweden to celebrate with Max and Shellback, my two collaborators on this album. Really we just wanted to reflect and take in how grateful we were for the moment we were in, but there was a studio there and we did what we do when we feel anything at all: We wrote more songs.”

She added, “I hope you love them, because they were born out of pure gratitude for your exuberance for and celebration of the album we made.”

The four-song expansion comes less than a year after Swift released The Life of a Showgirl, her 12th studio album. The album included hits including “The Fate of Ophelia” and “Opalite,” while Swift’s most recent original song, “I Knew It I Knew You,” was released in June as part of the Toy Story 5 soundtrack and became her 15th No. 1 single on the Billboard Hot 100.

The announcement also came shortly before Swift is set to receive the inaugural Artist Director Honors at the 2026 MTV Video Music Awards on Sunday, Sept. 27. The new award recognizes artists whose work behind the camera has expanded the possibilities of music videos and visual storytelling.

She has directed several of her own music videos and the short film for “All Too Well,” which won at the VMAs, as well as videos including “The Man,” “Cardigan,” “Willow,” “I Can See You” and “The Fate of Ophelia.”
